Extension question

Dear all,
My current Tier 1 expires on October 2012. My 5 years of stay finishes on March 2013. I will be extending for the second time before October 2012. I would like to get some clarification on the following questions

1) Since I only need 5 months extra time to qualify for PR can I apply a few months in advance i.e now? (I am fine even if they give me 3 years extension starting from date of application)
2) What form should I use from https://apply.ukba.homeoffice.gov.uk/iapply.portal ? Is it "Tier 1 General"?
3) In the above url I am not able to book any appointment/transaction at all. I choose the form, download it and what am I supposed to click next?
