J1- Have u lived at any other address in the UK within the past five years?

Scenario: 1) I Came to UK two years before if so do i need to give my India address to show five years or is it only within UK address?

Scenario: 2) Came to UK Jan 2007 and returned to India on Aug 2009, Stayed in India for a year, Then came to UK on Sep 2010 and currently staying here, So, Do i need to give Both India and UK address covering for five years or only UK addresses where stayed?
