ILR dependant question

Hi,
I have to apply for ILR in Sept 1st. I am on Tier 1 Visa and finishes five years in Sept 2012.

HSMP Entry Clearance: 28/Sept/2007
Entered UK: 25/Oct/2007
Tier-1 General Ext: 14/Sept/2009
Tier-1 Valid Until: 28/Sept/2012

My wife is on dependent visa
Entered into UK on 23/08/2008
went to India during pregnancy on 04/01/2011
Return to UK on 19/07/2011

I have few questions

1) Is My wife eligible to apply for ILR? because, she is more than 6 months out of UK and she has not at finished two years after her return (19/07/2011)?

2) Cohabitation evidence requirements :
She was not there in six months in last two years,
I have found following documents. Is it sufficient?
1) Tenancy agreement on 01/09/2010 with both names
2) Water bill on 11/09/2011 with both names
3) Council tax on 03/03/2012 with both names

3) One form of Set(O) Application sufficient to apply for me and my wife and two children?
