Booking / ticket reference number for first travel

Hi,

I am filling out visa extension form for tier 1 extension. On page 21 ' Tier 1 (General) Application form (version 04/12) Page 21 ' of the form it asks for

D8. Give details of the journey, including where the applicant travelled from, ticket booking or reference number, and transport operator.

I don't remember the booking reference / ticket number and I don't have any data available on line because I booked the ticket by visiting travel agents office.

What should I fill in this form?

you go to question D8 only if you have answered question D5 yes.

D5. Are either the applicant’s current and/or any one of his/her previous passport(s) or travel
document(s) that he/she has used to travel to, and remain in, the United Kingdom which
shows his/her current leave unavailable?

Are you sure answer to question D5 is yes in your case??

yeah I was wrongly looking at D8, I have answered D5 as NO and I should go to D11. Thanks for helping me out.
