How to book an appointment online? (SET-O)

Hi, I'd like to book an appointment online but it seems to be impossible to get to the stage where I can try and choose the date etc. Could you help?

I start from https://apply.ukba.homeoffice.gov.uk/iapply.portal, choose SET-O, get to stage 2.1 where I can download the paper form and cannot get to the next stage. There seems to be no clickable links to the next stages and there is no automatic redirection. I tried Chrome, IE and Firefox but no luck. What am doing wrong?

The webpage is poorly designed. Please look at the top right hand corner where you can see the links to either sign up if already registered, or register if you've not done so yet. I guess you want to register first, it will then follows automatically.

A point to note, I needed to download the paper form for SET O, unlike my earlier, a couple of months back, one for Tier2 applicaation, attempt where I could fill the form online.
