Urgent quick question - please help

Hi My husband retured last night from the biometric clinic - we have to post the docs from his country. They have given him a letter saying where the post the docs and also to ensure that he includes his passport, signed application form, supporting documents and a completed checklist.

I can't find anything on the UKBA website about a checklist that must be enclosed with a spouse visa application. Anyone know what this is?
