Belgium Shengun Visa

Hi,

We are going Belgium on holidays in December and I've to apply for shengun visa in Belgium embassy. As my wife is british so I'm exempted from visa fee but I can't find the right application form to apply for visit visa in Belgium embassy.

To apply as a spouse of my british partner do I need to fill up same application form twice firstly for myself and then filling up same form to mention my british partner.

I'm bit confuse here please someone can put me on right direction or send me a link to download the right application form.
