HELP !I m confused which application form should i send?

I’m an Moroccan national married an Slovakian national We live in UK more than 9 years , my 5 years Visa will finish in 2 month time ,My problem now is i m really confused to which application form should i fill for my indefinite leave to remain in the UK ?

I’m have printed SET(F) but i not sure about this application form.

Please help ,any advice will be much appreciated.
