Urgent Help Needed - Dividends

Hi All,

I applying for tier 1 general extension in coming week.

I am taking salary from one organisation and showing rest income from ltd company as director with 100% shareholder . I AM NOT REGISTERED AS SELF EMPLOYED WITH HMRC during EARNING PERIOD

I am little confused about following thins with respect to showing income as Dividends only from ltd company wherein I am 100% share owner.

1) Whether I should tick employed or both Employed and self employed.
2) I am submitting the following documents with respect to business account
a) Accountant letter with profit and loss account
b) Divident Vouchers
c) Persnal bank statement for net divident credit

3 ) Do I need to get letter from Accountant for exemption towards NI contribution

An early response shall be highly appreciated.
