URGENT - FLR(HSMP) URGENT TREATMENT

Hello Friends & Moderators,

I got my HSMP approval and sent my passports along with FLR(HSMP) forms on 22nd November. And I got a job offer and the company wants me to join them from 1st Jan 2007. Could someone please guide me what evidence I need to provide for the URGENT TREATMENT form. (Is mail from the new company sufficient or need to fax the offer letter as well).

Also would be greatful if someone can tell me if there is a separate URGENT TREATMENT form for FLR(HSMP). I could only find one for FLR(IED) and I am planning to use the same.
