Tier 1 General Dependant - Visa requirements

Hi

1. Can anyone please indicate list of documents which are required to apply for visa for my wife from pakistan.

2. Can anyone tell me about the maintenance of funds as i have been keeping 900 for myself and 600 for my wife in account. But i have been granted visa in Feb 2012 in appeal but my PSW was expired in feb 2011. So i am confused about maintenance but in all that period i was in UK and have been working since then.

3. Bank Statement should be no older than 30 days by the time of application?
