Country Codes and Income Bands

Hi,

Can anyone answer what needs to be filled out for Q28 and Q31 of the HSMP Application?

Q28 asks about relevant country code from the guidance notes. Do I put "United Kingdom" or "A" here? I can't see any country codes specified in the guidance notes other than the full country names specified in the income bands.

Q31 asks about the relevant income band from the guidance notes. I expect this is either "A" or "40,000+". I was going to put "40,000+", but then Annexe C, paragraph 35 says "there are 5 income bands". The are more than 5 options in the list of pay ranges...so I am confused as to what to put in for income band.

Basically, to make a long story short, can people tell me what they put in for Q28 and Q31?
