Urgent Filling T-2 Application(postal)doubt in Section C

My situation:

Switching from: PSW
No. of Passports: 1
Previous History: Entered UK as Student in Sept 2009 and applied for PSW valid from May 2011 valid until May 13.

In Section C - Passport or travel document (Page 11)

C1. Current passport or travel document:

Filled with present passport details.

another question under C1

Previous passport or travel document what should i fill in here?

C2. Does your current passport or travel document include a photograph of you? (Page 11)

I've answered YES for that.

C3. Do you have any passport or travel document which contains a previous grant
of leave or which you used when you last travelled to the UK?(Pages 11 and 12)

What should i fill here??

Another small doubt as well, I am from India. Do i need to register with police? because INDIA is not in that countries list provided in Appendix 2
but here

This guidance gives information on the police registration requirement of part 10 of the
Immigration Rules paragraphs 325 and 326.
When to impose the requirement to register
The requirement to register with the police is imposed on an applicant who is:
• a national or citizen of a country or territory listed in Appendix 2 to the rules
• a stateless person, or
• a person holding a non-national travel document.
And who is given:
• limited leave to enter the UK for longer than six months
• limited leave to remain allowing them to remain in the UK for longer than six months,
starting from the date of their arrival.

makes me think again.

Please help me experts, I got stuck in here.

You should fill YES, as u have a previous grant of leave as Student and PSW

Do i need to register with police? -- NO
