Tier 1 Visa extension doubt **urgent** please help

Hi,

I have my Tier-1 General extension day after tomorrow..I need help with following points.

1. On the Dependant form section K9 it asks for "What date was your visa or entry clearnace valid to": Which date should I put here i.e. the valid until date of the current visa or the valid until date of the 1st visa which was used when she came to the UK for the first time.
2. I have lost all my olr reference number..Hope this field can be left blank.
3. I am a full time employee is a employee reference letter mandatory?I now dont have time to get a reference letter.
4. I am submitting the following documents hope this is all we need
a) application form
b) 12 months original payslip
c) 12 Months bankn statement
d) photo graphs
e) Marriage certificate
f) DEgree Certificate
g) Passports and BRPs.

Current Visa Valid Until

Does'nt matter...Leave blank

How do prove UK experience? is Pay slip and Bank statement enough?

Also did I miss any other document?

It will be better to have your degree certificate to prove your english skills. Your pay slips and bank statements should be fine to prove you UK experience.

You dont need to prove your english skill any more...
The documents already submitted in previous application can be excluded from current application...
