Tier 4 & Dependent Application

Hello Everyone,
I am going to apply for tier 4 visa extension within a couple of months. Apparently, my wife, who is staying with me would also apply at the same time.

(a) I was wondering whether we can use the same application form or we need different application form? If we need to use different application form, can we enclose the total application fee and application form (mine and my wife's) in the same application pack? also

(b) My mother is going to sponsor all of our expenditures. Do I need to enclose a notary certificate as proof of sponsorship or what documents do I need to enclose as a proof from my mother's sponsorship?
