Help with Application Form

Hello All
I am from Chennai and will be applying for HSMP in next month. I have some questions with application form

The HSMP form asks for contact details for each employer including a named contact to confirm the information. Where should we include this information?

Calling all members who have got approval. Has anyone from HO called up your employer?

just write on a plain paper or submit along with ur covering letter

Also who would be a good contact. Is it HR, my manager or any colleague of mine?

Better to give the contact details of your immediate manager or HR
