Need Information on Visa Appeal Process

Does anybody know how to appeal if the Home Office refuses a visa extension? I've read that after they say no, they give 28 days to appeal. But what then? How long does it take them to respond? And if they still say no, can a second appeal be done? And if I'm trying to extend a fiance visa, would marriage be a valid response to their saying no?
