ILR - Form to use and letter from employers

Hi,
can somebody advise me on the following -

1. I am on a Work Permit, General Tier 2 BRP. Looking at the UKBA site, it seems I need to use the SET O. Can somebody please confirm if this is correct?

2. It states that I need a letter from employer to show they still intend to employ me. is there a format for this ? What exactly is required to be written on that letter?
