I will complete my five years in UK on a combination of WP +HSMP on 9th May 2007
(Initially was on WP --> then move to HSMP last year in April) .

May 2002 to March 2006 (WP)+HSMP(April 2006) valid until April April 2008

My wife is on Dependant HSMP.

We have been married since Jan 2006 .My wife landed in UK on 1st feb 2006 .

Which form should be used by me ?

Would there be different /same form would suffice or my wife's it should be different form .

What documents would needed to support my and my wife ILR .

You need to apply thru FLR(O). You can include your dependants in the same ILR form along with you, both will get ILR at the same time. I did it myself last year. You just need proof of marriage, and other documents specified in FLR(O).

Thanks for reply .Just was wondering I just went through the FLR(O) form but it states employment not requiring work permit .
Is it because I am on HSMP ?

Jeevan, I was under the impression that it is the SET(O) that should be used in such situations,i.e- when you apply for ILR. Are you sure that you used the FLR(O) and not SET(O)?

Thanks Jabi .I think you are right SET(O) form should be the right one .
With that what additional documents would be needed ?

Since she is your dependant, she should get her ILR with you as well. Make sure you get all the evidence as specified in SET(0).
