Pls Help Tier 2 extension

I have just few questions regarding tier 2 extension application.

2: will my employer need to apply for new CoS and what will be the process for CoS allocation for extension applications? What is difference between restricted and unrestricted.
3: what is the maintenance requirement?
4: is it a must to submit pay slips with extension applications? if yes then how many months? my first visa was granted for 2 years now can i apply for 4 more years visa?

2 your sponsor will need to assign a new unrestricted CoS to you which they'll do online (restricted CoSs are only for applications from abroad)

3 none

4 no payslips needed but check that skill level, SOC code and salary are in line with the requiremenrs. The max period of grant is 3 years or to the work end date stated in the CoS if that is shorter.
