Best time for Tier 2 G Visa extension ?

Hi Guys,

my Tier 2 G Visa which will be expired on Oct 2014. could you please guide me what is best time to apply (how many days/months before expiry)?
anything to care about at this stage (8 months before expiry)?

what about if my sponsor process it earlier , will the extension end before total of 5 years and i will face problem for ILR?
is there any case to get extension for 3 years or always get 2 years extension?
