Dependent Visa extension!!

Hi,

I am currently in UK on a Tier 1 general dependent visa which is due to expire in June-2014. My husdand(main applicant) got his ILR in march 2013( i had applied for my dependent visa prior to this in Feb 2013)
So now i need to renew/extend my visa and i am not sure if i need to renew my existing visa or get a new visa as a dependent of a settled person.

If i need to renew my existing visa then which form should i fill?
if i need to get a new visa as dependent of settled person from within UK then which form do i need to fill?
