need provide Police registration when apply for HSMP extensi

DO I have to provide Police registration certificate when applying for HSMP extension?
My address changed, but I didnot go to police station to change it.

My address changed, but I didnot go to police station to change it.
Isn't it a condition of Police Registration that you notify change of address to the Police? If so you better do that as soon as possible.

thx, is it a must to provide this document whilst apply HSMP extension?
