If I pay by Debit card, is debit card owner's sign needed?

If I pay by Debit card, is debit card owner's sign needed?

Can you help me in filling up these details :
I want to pay by debit card. Ref : item 11, 14 of the HSMP form in page 1.

Can you tell me :

Payer Name <is it the card holder's name or HSMP applicant's name ?>


AND :

Signature : <is it the card holder's sign or HSMP applicant's sign ?>

It's the owner of the debit card's name and signature - the HO wants legal authorisation to charge the debit card and only the owner of the debit card can give that authorisation.

Think of it this way: If you and your friend go shopping and you pay for an item with your friend's debit card, would you sign the payment slip or would your friend? :)

CITIBank Mastro international debit card -issued in India OK

I want to use my CITIBank -India issued International debit card - Mastro
to pay for the fees ?

Has any one used this kind of cards ? Do I have to select
Switch/Maestro (UK) option ( as per item 9 in page 1 of HSMP form) or not?

Also what is the Issue No (switch) == ? (as per item 13 in page 1 of HSMP form)
