Tax Ref for FORM AN

Hi

I am bit confused with some questions in the section 3 of FORM AN for naturalization.
Basically I have been working in two jobs (2nd job started 3 months ago).However, I received a letter from HM revenue & custom recently which shows two tax reference numbers for each of my two jobs.So, under question 3.5 should I write both tax reference numbers and under 3.3 write both employers' names?

Hoping for a reply from the experts.
