OCI application

Dear friends,
Given OCI process ambiguity, please could some one share his/her recent experience?

1. Do I need to book and appointment in advance or is its the old process like queue up and get the ticket at HCI London?
2. Can I submit applications for family (kids and spouse)?
3. Whats documents are required and how many photo copies?
4. How long does it take assuming I'll book the appointment in advance?
5. Any other check that I should take care of?

3. List is available on HCI London site, visit https://www.hcilondon.in/oci.html
4.6 to 8 weeks after submitting application
5.Make sure photo is correct size,while filling form, check all spelling correctly and other details as HCI will not be responsible for incorrect detail on card.

Just to add what nehalbhagat had said - don't forget to take the surrender certificate with you (original + copy)
