Tier 2 General - Baby born in the UK (Dependent Visa)

Hi

I would like to know the exact visa requirements for the baby born in the UK. Checklist which I have, please inform if I am missing anything:

- Letter from employer
- 6 months payslips
- 6 months bank statement
- Main applicant passport and baby's passport
- Birth certificate

How much money do I have to show for bank statement?
How much will it cost for the premium service?
Does my employer have to update my notes in SMS if I apply for my baby?

Please confirm fi there is anythign I need to show?
