Tier 2 (General) dependent and tenancy agreement

I know this has been discussed many times but I'm still not sure whether to submit my tenancy agreement for my wife's visa. I know its not an official requirement but I have heard that it all depends on the entry clearance officer and country of application (my wife is applying from Pakistan).

Has anyone actually been refused a dependent visa due to lack of tenancy agreement?

Has anyone had a successful application without submiting a tenancy agreement?

Please specify your visa category and country of application as well. Mine is Tier 2 (General).
