Tier 2 General extension

Hi

I am applying for Tier 2 General extension . My visa will expire on Decemebr this year. I would like to ask the following questions:
> New entrant or experience worker rate?
> do I need to provide bank statements?
> is there a need to change the job title?
> can I make an application online?

But the same Soc code to use based on the changes made by HO
