Transfer of EEA4 & change of name on EEA3

Hi,
can someone please advise how can i transfer my PR on new passport and change of name on Document certifying permanent residence card of Eu national.
I know it is not transfer , it is a new application and it is not necessary to transfer i can carry both old and new passports but still i want.
My eu spouse want to add my surname on her PR as she got new passport with my surname.
what documents should i send with new applications EEA3 & EEA4 ? i want to send both applications together.
and
after BC will PR status remain valid or it is not valid for travelling .??
