Right Form - Tier 2 General Extension + Dependant

Hi,

Please can someone confirm which form I need to use to apply for Tier 2 General - Extension?

but not sure which form to use on this page?

Can I include my dependent within my form?

Or do I need to fill a separate form for my dependent? Which one is it?

If I apply Online, do I still need to submit these forms?

You would need a separate form for the dependant if you are applying by post. If you are applying online all the details will be in one form.
