New form published again for tier-1 extension in Dec 2014

New form published again for tier-1 extension in Dec 2014

Does anyone know what was the new rules for tier-1 general migration extn application? I can see they have again published a new form on December 2014, the last one was published on April 2014.

Please can someone shed some lights? On the first page it is saying in accordance with Paragraph 34 of immigration rules.
