Urgent.. appendix required ?

Hi.

I have just completed my biometrics today for my tier 2 general visa. The document collection officer asked me about appendix as i have not included it with my supporting documents.
I want to confirm if it is necessary to include appendix ? I have completed my application online and i have not see any requirements of appendexes

The note at the end of App 5 reads:
YOU MUST SUBMIT A PAPER COPY OF THIS APPENDIX WITH YOUR SUPPORTING
DOCUMENTS. ENSURE YOU ALSO COMPLETE THE PERSONAL DETAILS FORM
EITHER ON PAPER OR ONLINE. YOU SHOULD SIGN THE DECLARATION AND
SUBMIT THIS WITH YOUR OTHER DOCUMENTS
