Tier 2 Main Applicant & Dependent Maintenance

Dear Experts/Gurus/Masters - Immigration Board

I'm going to apply my Tier 2 Extension(Change of employment) today or by tomorrow. Please answer me

1. Do I need to provide any maintenance letter(Proof of Evidence) along with the application? Note: My employer is A-rated Sponsor. I knew the maintenance of funds not required, but I'm asking for the dependent application.

2. If I book an appointment today for premium service (Appointment date available next: 8/April/2015), Still those salary high & medical rules apply??

Please advice. I appreciate if u answer this 2 questions as soon as possible.
