EEA Family Permit and Health Care Surcharge

Hey

We have just handed in our papers for re-application for EEA Family Permit, and were now presented with a requirement to go through the health care surcharge registration site, http://www.immigration-health-surcharge.service.gov.uk.

That seems fine, we will register and pay if necessary.

But how to register?
EEA Family Permit does not appear as an option.
Should we choose Settlement?

And does anyone know anything about the payment methods?
We don't have credit cards with the required limit (bloody Filipino bank system....).
Can these surcharges be paid at the embassies?

Hope you can help, as we were told to register within 24 hours.

Have also written to the embassy, but not confident they will get back to us within 24 hours.
