Details of English Certificate in AN Form

I am confused so maybe someone can help me out.

The AN form requires me to enter

Title of qualification
Level of qualification
Award reference number

The certificate from the Trinity College says

Grade 5
Graded Examination in Spoken English
Entry Level Certificate in ESOL International
(Speaking and Listening) (Entry 3)
CEFR Level B1.1 with distinction

At the bottom of the certificate I have three numbers

Qualification Number
Trinity ID
Candidate Number

Which part is now the title / level and reference number ?

There isn't much room to add the whole lot so would be the title "Grade 5 GESE" and Level "CEFT Level B1.1" and what about the reference number ? Would that be the "Qualification Number" ?

The email received from the college mentions that the reference number used is in the format

TCL/200xxx/058xxx/5000xxxxx

There is no way this fits in the field on the AN form as it only allows 14 characters ...

Thanks, actually I am an idiot (or the AN just confusing at times).

I thought that initially, too - that the "TCL" is the right one, but the 14 character reference field confused me. But that field was part of the certificate received BEFORE April 2015 .. so all clear .. thanks...
