Visitor in transit Visa: help required please

An Indian friend of mine is travelling from the USA to India via London heathrow.

1. His 1st leg of the flight is USA to London heathrow.
2. His 2nd leg of the flight is from London heathrow to India.

There is a visa flavour called 'transit visitor visa' to come out of the airport which I think he'll be eligible to apply for - because his onward journey to India where he is a citizen is well within 45 hrs. All good.

Questions are:

1. Does he have to apply for this 'transit visa' once he lands in the London airport ?
2. Does he have to apply for this 'transit visa' online over the internet when he's starting from the USA ?

1. No, he has to apply before he flies out of the USA.
