Required Documents for 'Family of settled Person'

I and my wife, we both have ILR. My child was born 6 months before in India.
I am applying 'Family of settled Person' visa for my child.

What are the documents and appendixes I need to submit along with the application ?

I was reading through the documents.

What kind of proof I need to include for my income ?
Do I need to show my last 12 source of income ?

I was working in a company until December 2014. After that I started my own limited company.
However, I started to take minimum salary only from April 2015.

Could you please guide me what i can show for my income ?

Do I need to fill in Part 1 of Appendix 1.

It says - "Do not complete this part if you are applying as the child of settled parent(s) with limited leave"

Can someone be settled with limited leave ?
