Professional Refree signature

Hi there

I want to ask a question regarding signature of professional referee.
Is it important that a professional person should know the applicant ONLY in the professional capacity? Or it can be anything else as well?

Like can a professional referee write that the applicant is his/her "friend"? Or it has to be any professional relation?
