Tier 2 Dependent Checklist

Hi All,
I had applied for my visa extension in the month of April and now i am planning to do the same for my Kid who is residing in India.Could you please provide me the checklist that needs to be submitted for this.
1) Depedent 2 Photographs
2) Dependent Original Passport
3) CoS Letter of Main Applicant
4) Letter from Employer with the Dependent details
5) Letter from Parents as they are outside India
6) Tenacy Agreement
6) Bank Statement with Funds for last 3 months

PLease let me know if i am missing anything here.
Additionaly i was on a unpaid holiday for the last 45 days so is it mandatory to show the salary getting credited in the given Bank Statement or maintating funds is okay?
And also please confirm me if the Payslips are mandatory for the application.

You can consider to enclose these -

1. Your child original birth certificate
2. Payslips
3. Mention about your 45 days leave in a covering letter
