It doesn't display properly here but put your address on right-hand side of the letter:

<your contact phone number>
<date of letter>
Continue with rest of letter...
FAO: <caseworker name (if known)>
<HO address>
<town>
<post code>
Reference: <add any relevant HO reference number you may have>
Dear <caseworker name, if known>
OR
Dear Sir/Madam,
Re: Vary Flr(FP) to Set(LR)

Re = Regarding
Use
Yours sincerely if you write to someone by name;
OR use
Yours faithfully if you just used
