Name diff b/w bachelors degree & rest of the documents

Hi Guys,

I need some advice on my situation.

The name on my bachelors degree is "Ayaz Ahmed Khan"
& on rest of my documents including passport / Salary Slips /\Bank Statements is "Ayaz Ahmed"

Please advise if this can become an issue for HSMP application?

Hi Ayaz,

You can find various posts in recent days on this issue.
I think better solution is to get an affidavit for that and get it attested from notary public stating that your actual name is "Ayaz Ahmad" but on degree its erroneously written as "Ayaz Ahmed Khan".

BTW - whats your name on other educational docs like 10, 12 grade certificates.

The name on my metric degree is the major cause, over there it is written "Ayaz Ahmed Khan" and in FSC(12th Grade) it is "Ayaz Ahmed"
and for my bachelors degree the checked my metriculation certificate and thats why they wrote "Ayaz Ahmed Khan" on my Bachelors degree.

If I have to change my name on Bachelors degree then first i have to change the name on my matriculation certificate and that process is very long.

Is there any format of letter for this kind of affidavit? Affidavit on stamp paper or plain paper will work?
Moreover, Can i do this affidavit from country other than my home country i.e. currently i am staying in SouthAfrica and my home country is Pakistan.

I'm not sure about format but think that u can do from SA.

Just search around the forum and other websites. Also, contact som notary public there or any lawyer.
