If one has to apply for FLR M under following circumstances:

1. Spouse had the below visa categories before opting for the British Citizenship: HSMP --> Tier 1 ---> ILR ---> British Citizen.
2. If spouse is a NHS staff or becomes a NHS staff

This being the situation, is Immigration Health Surcharge (IHS) applicable for the FLR M ?

Not sure what you're asking but working for the NHS doesn't mean you don't pay the IHS surcharge.
