Please any one help.... Issue with marriage certificate

In the first page of our marriage certificate my wife name is same as before our marriage, she has used her family name and father's name as initials. But in the second page of our marriage certificate she has used my name and my family name as initial since she thought that after marriage it is a practice to use husband's name as intitials. But in both the pages of the marriage certificate her first name is the same and spelled correct. Only her initials are printed different. All her other documents have her father's name and her family name as intials. Infact she has included my name in her passport also.

Will this be of any big issue while obtaining visa??? Should I have to get an affidavit saying that both the names with different initials refer the same person?

Any one has renewed their Indian passport from UK? If so and if spouse name inclusion is needed in new passport, will spouses' passport with (husbands name already included in it) is enough? or again marriage certificate is needed? My spouse already used the existing marriage certificate (which has issues told above) for inclusion of my name in her passport. I am planning to register my marriage again and get a fresh certificate as the original one has propblems, if I use this new one will it be a problem at the time of renewal of my passport? Any legal issues?
