TIer 2 dependent: which applcation form should be used?

Dear All,
I will be applying for ILR as i complete 5 years of my tier 2 VISA later this year. My husband was granted 2 years leave as PBS dependent which expires in August 2016. I have read the new changes on UKBA website and am really confused. Which form should i use for extended my husband's VISA? Does he have to apply for another 5 years before he can be eligible for ILR?
I would be extremely grateful if someone can erply to this post ASAP.

He needs a 3 year extension to make up 5 years in total before he can apply for ILR.

He will also have to pay the immigration health surcharge.

If I choose the standard service when and how should I submit my Biometric?

if you choose standard service you print and post the form and you will get letter for biometric to got to postoffice. you will find all info in the same link.
