Hello,
I am asking this question for my aunt.

She has had ILR stamped on her pakistani passport since 1993. Her husband now wants her to get a british passport.

They are confused on what they need to do and how to go about applying for one. Will she need to pass life in the uk test/some sort of english speaking exam first?

Any help regarding this would be greatly appreciated.

She'll need to become British before she can apply for a passport.

She will need an English language qualification (unless she's exempt) and will also need to pass the Life in the UK test.

How old is your Aunt? Certain groups are exempt from LIUK and English test.

She has to apply on form AN and pay the £1236 fee. She needs to meet ALL the requirements (absence, good character, etc).

Once approved and she has attended her ceremony, then she can apply for a British passport.

Where does aunt live now?

If its in UK, has aunt had any prolonged absences from UK in the intervening 23 years or so?

She is in the uk. She's 48 (i think!) And no she hasn't had long periods of stay outside of the uk. Dont think she's even travelled anywhere the past few years.

Okay so firstly she will need to take life in the uk test AND pass an english speaking exam such as B1? Then apply for british citizenship, correct?

Yes.

She needs to do the English test at one of the approved test centres, see link below.

Once she has her LIUK and B1 English test done, she can apply on form AN for naturalisation provided she meets all the rules, including being physically present in the UK 3/5 years before on the same date of applying.
