Different Names in the evidences

In Birth Certificate, Degree Certificate, Driving License Name is as Ashik A S

In PAN Card, Employee Record is as Ashik Siddique ( they have expanded the Initials)

In Passport ,

Surname : Mohamed Abubucker Siddique

Given Name : Ashik

Which Name does i have to fill in the HSMP Form? And is it ok if i get an Affidavit from Notary stating that all r the same...

HI ashik,

Even i too have the same issue. If your name spelt differently in your documents it is always good to have notary stating all these names represent same person.

In company pay slip & bank statements : Satheesh Kumar Madurai Sundaresan

So, i think it will be good (as you said) to get a notary attestation.

And, also what should be the content of the letter for this attestation?

I have same name in certificates, but then in the pay slips and tax returns, the initials are expanded. So, was wondering whether that is accepted.

Any idea as to whether people have got approvals with expaned initials in the pay slips & with initials in the degree certificates?
