Who's name to put on the online declaration on VFS site

Hi,
I am applying for a UK visa for my daughter (2 months old) and have filled up the online application form on the VFS site.

After completing the form - it asks for a declaration to be signed.

There is text box for signature in which we need to enter a name.

The signature on the hardcopy form will obviously be mine or the childs mothers.

However I wanted to know whose name to put on the online declaration "signature" box - the babies name or the name of the person who would physically sign the hard copy form?

Can anyone who has submitted such an application please help.
