Where to put IHS number-Out of country Tier 2 Dependent visa

Hi guys

My husband is currently filling the application on the visa4uk website for a Tier 2 Dependent visa from the US. However, there is no place for him to put in the IHS reference number anywhere. Does anyone know where this goes? Shall we print out the payment confirmation page/email and attach that with the application?

Also, does anyone know if he will be able to select a biometric centre (at a DHS Application Support Centre) in another state? He lives in Colorado but would like to do the biometrics when he visit his sister in Atlanta as its more convenient.

Would appreciate any insight!
