Query in HSMP Application form

Hi,

What do I need to fill in the following fields. I am currently in Uk on work Permit.
1) Any Pervious home office number
2) Name & address of legal representative

Any previous HO Ref Number: I filled in my WP number

Name & address of legal representative : Leave it blank if you are going on your own

thanks for providing the details.. btw did you print the application and filled the details or filled the soft copy then took the print.. I know it is not going to matter any ways.. but just wanted to know how this done generally ..When I tried to fill the softcopy, I was not able to fill some of details in payment section ..did you have same issue??
